liver failure to convert ammonia into urea?

If the Kidney and liver seem to be functioning ok. (standard tests confirm)
What would be causing ammonia odor of body fluid/urine.
Is there something else doctors should test for?
also, could intestine problems somehow be a factor?

PS: I have researched this and am at dead end.
My only thought is it is arginine deficiency.

either no one knows the answer like me or I asked a dumb question
Its kind of important as these are real symptoms of someone I know whose health is deteriorating , and primary doctor does not know or care to find out the answer either. Well google turns up nada so I must go to the library to research perhaps in med books
